GEO, AEO and Local SEO: same Google rules, a separate measurement surface

Clinic and SME owners keep hearing two lines. One: good Local SEO is still enough for Google. The other: without GEO or AEO you stay invisible in AI answers. Both undershoot. Search Central says AI Overviews and AI Mode use the same technical requirements as classic Search.[1][2] Google and Bing still measure citations in generated answers in their own reports, not only as a Local Pack rank.[3][9]

The short map

This piece sorts that tension. It does not promise clicks or medical outcomes. The lead path here is visibility and audit.

Three acronyms, three jobs.

Local SEO is the foundation: a current Google Business Profile, a consistent address, opening hours, LocalBusiness markup where it helps rich results.[4][5][10] For clinic pages, start from clinic solutions.

AEO (Answer Engine Optimization) and GEO (Generative Engine Optimization) are market terms. Google writes that many of the sold «hacks» are neither effective nor supported by how Search works.[2] In the research, GEO is narrower: visibility as a citation or use inside a generated answer, not as a blue-link rank.[6][7]

Local Pack positions alone do not show that new surface. The other way around, buying a GEO toolkit while the profile goes stale often drops the eligibility everything else sits on.

What Google actually requires for AI Overviews

The eligibility rule is short. The page has to be indexed and allowed to appear in Search with a snippet. «There are no additional technical requirements.»[1] The same foundational SEO practices apply. Google says no special optimization is needed for AI Overviews or AI Mode.[1]

«Is SEO still relevant for generative AI search?» The optimization guide answers «In short, yes!».[2] The generative features sit on the same ranking and quality systems.

Google does not support chunking tricks, inauthentic mentions, or an extra schema only for generative search. Structured data is not required for generative AI search, and there is no special schema.org markup for it.[2] LocalBusiness still helps rich results; required fields there are name and address.[4] Different job.

For local businesses, Google still lists a current Business Profile among the basics that are worth doing.[1][5] Maps and the Knowledge Panel run through the verified profile. An AEO hack does not replace that.

What Google and Bing still measure separately

On 3 June 2026 Search Console announced dedicated Search Generative AI performance reports. They show impressions in features such as AI Overviews and AI Mode, first for a subset of sites.[3]

In February 2026 Bing Webmaster Tools opened a public preview called «AI Performance». «visibility is not only about blue links», it says. The question is whether content gets cited in Copilot and AI summaries. Bing calls that an early step toward GEO tooling.[9] For location-based AI answers Bing stresses current business info and Bing Places.[9]

Own measurement surface, own object. Local Pack rank still matters. It is not the same as «was this page named in a generated answer?».

What papers mean by GEO (and what «40%» is not)

Aggarwal et al. introduce GEO as «the first novel paradigm»: visibility in generative-engine answers, with its own visibility metrics.[6] The same work appeared at KDD 2024 (DOI 10.1145/3637528.3671900; issued 24 August 2024).[11] Chen et al. write that the shift to synthesized, citation-backed answers makes SEO necessary but not sufficient, and they also call that GEO. Local AI search, they say, needs strategies «beyond traditional Google Local SEO».[7]

«By up to 40%» GEO can boost visibility in generative-engine responses, Aggarwal et al. report in their own setting.[6] Relative measure, paper setting. Martinez read 45 studies (November 2023 to July 2026) and rejected the figure as a general claim: the gain holds when the source is already in a fixed context. None of the reviewed techniques showed a stable, longitudinal, cross-platform causal effect on organic discoverability or downstream behavior.[8]

Academically, GEO is a different target than rank. «40% more traffic» does not follow.

Local SEO stays the foundation, not the whole job

schema.org defines LocalBusiness as a particular physical business or branch. One type example is a medical practice. That is a class label, not a medical claim.[10]

Google requires name and address among other fields for local-business rich results. It explains the Knowledge Panel and Maps through the Business Profile.[4][5] Bing ties local AI answers to a current address, hours, contact, and Bing Places.[9]

Without that foundation, eligibility is often already missing. With it alone you still cannot see whether an AI answer names you. Once the property gets the reports in [3] and [9], measurement lives there.

What owners do now (without hacks)

Keep Business Profile and Bing Places current: address, hours, contact, category. That stays the base.[5][9]

Keep pages indexable, with a clear snippet. Do not invent extra technique for AI Overviews.[1][2]

LocalBusiness markup is worth it for rich results if the page is a local business. It is not a GEO spell.[2][4]

Look in Search Console for the Generative AI reports. If they are missing, the property is probably still outside the rollout set.[3] Same for Bing AI Performance.[9]

Paper percentages are not a traffic guarantee.[6][8]

New since 31 August 2026

On 31 August 2026 Google released two things to all websites worldwide. The generative AI performance report in Search Console now shows how often links to your site appeared in AI Overviews and AI Mode, broken down by pages, countries, devices and date range.[12] And a property-level control lets you exclude your site from those generative features or include it again; the default is inclusion.[13]

With the report, what it measures deserves a close look: impressions. Clicks, click-through rate and position do not appear in it. So anyone talking about measuring AI citations is, at Google, currently counting AI visibility. Attribution goes to the canonical URL after redirects, Search Labs experiments are excluded, and the same row limit applies as in the regular performance report.[12]

The control is more precise than it sounds. An exclusion prevents both the linking and the use of your content as grounding for generated answers. Google states explicitly that this control is not a ranking or inclusion signal for the rest of Search, and that it does not affect AI training; Google-Extended covers that.[13][14]

The split between search and training runs through all four providers. Blocking every AI bot wholesale also blocks the visibility you actually want.

The most common accidental self-exclusion

Beside the new control there is an older, quieter way to disappear from AI answers. Google's robots meta tag specification states that nosnippet applies to all result formats, AI Overviews and AI Mode included, and prevents the content from being used as a direct input for those features.[18] A max-snippet value set too tightly works in the same direction.

In practice such directives often come from an old precaution against snippet scraping, or from a template nobody reads any more. So checking whether your clinic is even allowed to appear in AI answers starts in three places: the property control, the robots directives on the page, and the bot rules in your firewall or WAF.

Frequently asked questions

What is the difference between GEO, AEO and SEO?

Google places the terms itself: AEO stands for answer engine optimization, GEO for generative engine optimization, and from Google Search's perspective, optimizing for generative features is still SEO.[2] The technical requirements are the same as for classic Search.

Do AI answers require special schema or an llms.txt file?

No. Google states explicitly that generative AI search needs neither new machine-readable files nor special schema.org markup.[2] Structured data remains useful for rich results, but it is no entry ticket to AI Overviews.

How can I measure whether my site appears in AI answers?

Since 31 August 2026 the generative AI performance report in Search Console shows impressions from AI Overviews and AI Mode worldwide.[12] It contains no clicks, click-through rate or position. Bing runs its own AI performance report.[9]

Can I keep my website out of AI answers?

Yes, through a property-level control in Search Console. The exclusion prevents linking and use as grounding, takes effect within a day or two, and according to Google is no ranking or inclusion signal for the rest of Search.[13] It does not govern AI training.

Does blocking Google-Extended block my AI visibility?

Not in Google Search. Google-Extended governs training and grounding in Gemini products and is, per the documentation, neither relevant to inclusion in Google Search nor a ranking signal.[14] ChatGPT, Claude and Perplexity each use their own crawlers.

Why does my clinic not appear in AI Overviews?

The most common causes are technical: the page is not indexed, may not show a snippet, is excluded through the property control, or is blocked by your own firewall.[13][18] Content reasons come after that.

Is a purchased GEO package worth it?

Check what is in it first. Chunking tricks, bought mentions and special markup for generative search find no support in Google's own documentation.[2] What carries weight is indexability, snippet eligibility, a maintained profile and evidenced content.
